Thank you for being willing to adopt an animal!

Before filling out this form, please be sure you have read our adoption policies:

  • You must be over the age of 18 to be considered to adopt a pet.

  • All of our animals are brought up to date on vaccinations, wormed, and will be spayed or neutered before going to their new homes.

  • There is an adoption fee that helps us offset the expense of veterinary care and getting these animal ready for new homes.

I agree all of the above statements are true to the best of my knowledge. I understand it takes time for a pet to adjust to a new environment and agree to give the animal a few weeks to become comfortable in my home. I also agree to return this animal to IPAW should the animal not be the right fit for my family. I will not surrender or abandon this animal.

Once your application has been approved, we will contact you to set up a home visit with the animal that you are interested in adopting.
